Safety Reps Bullying Campaign - Risk Assessment Form

The Risk Assessment Form is a tool to use in reporting Bullying incidents.

On the Risk Assessment Matrix part of the Form the risks are listed in severity (consequences x likelihood)from 1 to 6, with 6 being a minor risk and 1 being the most severe. In many cases of bullying 1 would be selected i.e. bullying can lead to severe ill health or worse.
